Could you add this line
puts @loaded_stacks.inspect; puts gem.name
right above line /home/.rvm/rubies/ruby-1.9.2-p0/lib/ruby/1.9.1/rubygems.rb:228, run this again, and paste the output here. Also, could you give us the output of "gem list"?
